*You will be charged at time of booking. If there are any technical difficulties,The provider will reach out via telephone in an attempt to complete the visit. If the visit is unable to be completed, a 50% refund will apply, however this does not apply if no contact is made.
*After booking, you will receive an email to sign up to the patient portal. You will also receive pre-visit questionaires that include a consent form. Please have these filled out prior to the visit for a more efficient service.
